2017 Scholarship Application Form

To be eligible for a TANDF scholarship, a student must be:

  1. Enrolled in at least 6 semester credit hours or be a full time student in an ACEND approved or accredited dietetic supervised practice program for the upcoming school year (fall 2017 and spring 2018) or awaiting acceptance for one and;
  2. A member of the Academy of Nutrition and Dietetic (AND) and;
  3. Reside in Texas at time of application. Must have attended a college or university in Texas in the past or plan to use the scholarship fund to attend a college or university in Texas in the next academic year and;
  4. Undergraduate students must be enrolled in an ACEND program and have completed 60 hours or documented junior status and;
  5. Graduate students must have a Dietetic Verification Statement from a Didactic Program and;
  6. DTR must have completed 12 hours (3 in nutrition, food service, or food science) and;
  7. Must have a 2.75 overall GPA and a 3.0 GPA in major courses.

a. List all colleges, universities, or other ACEND* accredited programs attended or currently attending, with most recent listed first. (*Accreditation Council for Education in Nutrition and Dietetics)


b. If you are already enrolled in the program for which you are requesting a scholarship, list the expected date of completion/graduation (include month and year).
If you are not currently enrolled in the program for which a scholarship is requested, list programs (dietetic internships, coordinated programs, dietetic technician, colleges or universities, etc.) you have applied to, intend to apply to, or have been accepted into for the upcoming year (next fall and spring).

4. EMPLOYMENT/VOLUNTEER WORK HISTORY

List all employment and volunteer work experiences in the past 5 years with most recent first. Indicate if the experience was paid or volunteer. Briefly describe key responsibilities. When indicating the amount of hours, use Hrs/Wk for reoccurring experiences or Total Hours for limited time exposure. Note: Priority is given to experiences from the past 5 years, however, if you have professional work experiences from over five years ago, you may include it.

5. COURSES *

List all courses in foods, nutrition, food sciences, nutrition and disease, and food service systems from all schools attended. Grade Point Average must be based on or converted to a 4.0 system.

(Note: Completion of this section is required to validate your application. All students must be full time for the upcoming fall [2017] and spring [2018] semester to be eligible for a TANDF scholarship.)

2. Your financial resources (amount of student loans, ability to work and anticipated income from work, sources of support, income from parents/spouse/others, other grants or scholarships or fellowships, previous savings, previous work, etc.)

9. AGREEMENTS: *

In submitting this application form, I agree:

  • that I meet the qualifications for application and will be enrolled for at least 6 semester credit hours or be a full time student in an ACEND approved or accredited dietetic supervised practice program in the upcoming fall and spring semesters;
  • that the information contained in this application form is true to the best of my knowledge;
  • that, barring unforeseen circumstances, I will continue my plans for study as indicated in the application;
  • that my application will not be considered for a scholarship if it is incomplete and/or directions for completion are not followed;
  • to abide by the following Forfeiture of Scholarship Clause:

Texas Academy of Nutrition and Dietetics Foundation (TANDF) Scholarships are awarded for a nine-month school year with funds awarded in the fall semester of a given year, or in the case of a supervised practice program without semesters, are awarded at the discretion of the TANDF Executive Board. Checks are sent to the university or institution on behalf of a student recipient and are applied to the student’s account. If a qualified student is enrolled in a program which does not require tuition or the student already has a full tuition scholarship and does not have an account that the funds can be put in to use for room, board, or books, the scholarship check, paid to the order of the student, will be mailed to the Program Director who will hand-deliver the scholarship check to the student. Recipients must be enrolled in an ACEND accredited dietetic education program or a qualifying graduate program as outlined in the TANDF Standing Rules (TANDF will communicate the enrollment requirement in writing to a student in a program with a nontraditional school year. The student must meet those requirements.). Students who fail to meet these criteria will forfeit their scholarship, and the funds will revert to the TANDF treasury. Students who forfeit a scholarship must reapply to be considered for future scholarships.

I, the undersigned, have read the TANDF Agreements and Forfeiture of Scholarship Clause and understand that my failure to meet the above criteria will result in forfeiture of a scholarship if I am selected as a recipient. I, the undersigned, agree to indemnify and hold harmless the TANDF, its officers and its advisors from any claim, action, liability, loss, damage or suit arising from the execution of duties as outlined in the organization’s Bylaws and Standing Rules.

The application form and all documents listed below (except official transcripts) should be completed online by December 1, 2016. The student applying for a scholarship will include the following:
  • Original completed TANDF Scholarship Application Form
  • Scanned copy of your AND membership card
  • Letter describing short-term and long-range educational and career goals
  • Completed TANDF Reference Forms from three references. Obtain references from academic advisors, teachers, dietitians, and/or work supervisors familiar with your professional and educational ability/experience and overall potential for becoming a contributing member of the dietetics profession (Personal references from other sources are not acceptable).
Official transcripts from the TWO most recent universities or colleges attended with ACEND programs, including the official seal of the institution, must be mailed to: Anna Bramanti MS, RD, LD, CDE Foundation Director 2307 Crawford Street Houston, TX 77004 *Electronic official transcripts will also be accepted if this is required by your university. The electronic official transcripts must be emailed to director@tandfscholarships.org.

All application materials, both emailed and mailed, must be postmarked no later than December 1, 2016.

Incomplete applications will not be considered. Failure to follow directions will result in disqualification.

A confirmation email will be sent to you once the director receives your application materials.

Applicants will be notified in February 2017 and scholarship money will be applied in Fall 2017.
